Graves Gilbert Clinic and Humana Secure Long-Term Partnership
Agreement strengthens access, cost transparency, and quality care across south-central Kentucky
BOWLING GREEN, KY — (January 15, 2026) — Graves Gilbert Clinic (GGC) and Humana announced today they have reached a two-year agreement ensuring GGC will continue to care for and manage Humana-insured patients across south-central Kentucky. The agreement secures uninterrupted access to GGC’s full range of primary and specialty care services for Humana members and preserves existing provider networks and in-network benefits.
The new contract, effective March 1, 2026, and extended for two years with annual renewals, resolves prior contentions and aligns both organizations around shared goals: improving patient access, coordinating care, controlling costs, and ensuring high clinical quality. Under the terms of the agreement, patients will retain in-network coverage for appointments, procedures, and ongoing care at GGC locations without the need to change providers or seek new referrals.
“This new agreement reinforces our commitment to patients across the state and secures our relationship with Humana as a real partner,” said Mike D’Eramo, CEO of Graves Gilbert Clinic. “By working collaboratively with Humana, we are supporting better care coordination, improved cost transparency, and high-quality outcomes. Our entire team looks forward to continuing to serve Humana members with the same standards of clinical excellence and compassionate care they expect from GGC.”
Humana leaders echoed the shared commitment to continuity, access, and coordinated care.
“For the thousands of Kentuckians who rely on Humana Healthy Horizons, we are grounded in the goal of helping them achieve their best health,” said Jeb Duke, President, Humana Healthy Horizons – Kentucky. “Our partnership with Graves Gilbert Clinic is important to delivering on this commitment, and we look forward to continuing our relationship and shared goals of coordinated, value-based care.”
“We are committed to helping Humana members achieve their best health through coordinated, holistic care,” said John Litting, Humana’s Regional President. “We are pleased to continue our relationship with Graves Gilbert Clinic and continue providing the people we serve in Southern Kentucky with uninterrupted access to the integrated care they deserve.”
Both organizations will collaborate on initiatives to improve care coordination, expand preventive services, strengthen telehealth, and enhance chronic disease management programs. These efforts aim to reduce unnecessary hospital admissions, support more effective care transitions, and help members manage healthcare costs while receiving comprehensive care close to home.
Patients covered by Humana should experience no disruption to scheduled care. Both organizations will provide timely communications to members and staff to clarify benefits, appointment scheduling, and administrative details. Patients with questions may contact GGC Patient Services or Humana Customer Support for assistance.
