Beneficiary Notification Follow-up Communication

Medicare Beneficiary:

This is a follow-up correspondence to your receipt of our Accountable Care Organization (ACO) letter that notified you that our practice participates in the Medicare Shared Savings Program. The goal of the program is to reduce medical waste and cost of care for Medicare beneficiaries by increasing care coordination and improving the quality of care we provide. If you have any questions about our participation, please contact our office.

As a reminder, Medicare protects the privacy of your health information. If you do not want Medicare to share information with your health care providers for care coordination, please call 1-800-Medicare (1-800-633-4227). Medicare may still share general information to measure
provider quality. For more information on how Medicare may use and give-out your information, visit Medicare.gov and search “privacy”.

As insurance payers shift toward value-based care provisions, our practice wants to emphasize the importance of preventative care for our patients. As a primary care patient of our medical practice, we expect our patients to abide by the advice of our healthcare providers and remain compliant with any medical screenings and procedures you qualify for. If you have any questions about what screening you may be due for, please reach-out to our office with your inquiries
and to schedule. Your healthcare provider will also attempt to notify you when you are due for a preventative health screening.
