Treatment of Work-Related Injuries/Illnesses

GGC Employer Services focuses on getting employees in quickly and communicates with the employer and their worker’s comp. carrier. GGC Employer Services will take care of the employee, try to get them better and back on the job as soon as possible for minor work injuries.

DOT Medical Exams

GGC Employer Services is the area’s leading provider of work related physical exam services. All GGC Employer Services are certified and listed in National Registry of Certified Medical Examiners (NRCME) complying with all DOT standards.

BEFORE you arrive for your DOT exam

Required Identification & Documentation

  • Valid government-issued photo identification (e.g., driver’s license, passport)
  • If international driver, must bring valid photo ID (e.g., foreign national driver’s license, U.S. visa or immigration document)
  • Current Medical Examiner’s Certificate (MEC), if applicable (for renewals)

Medical History and Related Information

  • Detailed medical history, including:
    • Chronic or recent medical conditions
    • Surgical history
    • Hospitalizations
    • History of tobacco, alcohol, or drug use
  • Comprehensive medication list, including:
    • Prescription and over-the-counter medications
    • Dosages and prescribing physician contact information
  • Documentation from specialists if applicable:
    • Cardiology (e.g., for heart disease, stents, arrhythmias)
    • Endocrinology (e.g., for diabetes, especially if insulin is used, will need Insulin- Treated Diabetes Mellitus Assessment Form, MCSA 5870)
    • Psychiatry/Neurology (e.g., for seizure disorders, mental health conditions)
    • Sleep Medicine (e.g., for obstructive sleep apnea)
  • CPAP compliance report (minimum 30-day printout, prefer 90 days) if treated for sleep apnea
  • Recent A1C results or blood glucose logs for individuals with diabetes (we are able to do if results not available)

Assistive Devices (if applicable)

  • Corrective lenses or contact lenses (required for the vision portion of the exam)
  • Hearing aids (must be used during hearing test, if prescribed)
  • Any other assistive or medical devices used regularly (e.g., insulin pumps, pacemakers – bring device cards if available)

Examination Components – What to Expect

The certified medical examiner will perform or evaluate the following:

  • Vision test – must meet FMCSA standards (at least 20/40 in each eye, with or without correction)
  • Hearing assessment – whisper test or audiometry
  • Blood pressure and heart rate
  • Physical examination – including neurological, cardiovascular, musculoskeletal, and general systems
  • Urinalysis – required to screen for specific gravity, protein, blood, and glucose (Note: This is not a drug screen unless requested by the employer)

Additional Documentation (As Needed)

  • Previous DOT medical certificates or examination reports (if available)
  • Employer documentation outlining job duties or functional requirements (if relevant)
  • Skill Performance Evaluation (SPE) Certificate – if applying with limb impairment or amputation
  • Medical exemption letters or FMCSA exemption documentation, if previously granted

Conditions That May Require Further Evaluation or Disqualify a Driver

  • Uncontrolled hypertension or diabetes
  • Vision or hearing that does not meet minimum requirements (without correction)
  • Active seizure disorder or history of epilepsy (unless cleared)
  • Substance abuse history or recent DUI violations
  • Use of certain medications (e.g., opioids, benzodiazepines) without appropriate clearance

Medical Surveillance

Whatever the size of your workforce, GGC Employer Services can help write, enact and monitor a drug free workplace policy and testing process; including testing that complies with Kentucky’s Drug Free Workplace program.

Medical surveillance can include:

  • Breath Alcohol Testing (BAT)
  • Urine collections (5 or 10 panel)
  • Point of Care Testing (POCT) (renders results in less than 15 minutes)
  • Random drug screening
  • Random pool selection and maintenance

All lab-based test results include professional interpretation and specification to job applications.

Audiometric Testing

Employees who work in areas that fall under OSHA Occupational Noise Exposure standard, 1910.95, should be given a baseline audiogram upon initial hiring, with audiograms given annually thereafter. GGC Employer Services can travel to the employer and conduct the testing with a CAOHC certified occupational hearing conservationist. Testing includes a hearing history questionnaire and the hearing test administered in a sound reduction booth. If the testing shows that the employee has an unusual result or has sustained a standard threshold shift, a licensed physician or audiologist needs to review the test.

Physical Abilities Testing

GGC Employer Services offers Physical Abilities Testing for post-offer, pre-placement or fit for duty.

Testing completes a collection of multiple strength and range of motion measurements in as little as 30 minutes with results made available to employers.

Physical Abilities Testing helps:

OSHA Respirator Clearance, Mask Fit & PFT

GGC Employer Services can provide clinic-based or on-site OSHA Respirator Clearance. Medical data is collected and the OSHA Respirator Clearance questionnaire is reviewed by a licensed health care professional. An exam and Pulmonary Function Test (PFT) may be needed based on the data. All steps are done according to the OSHA standard, 29 CFR 1910.134.

GGC Employer Services also provides qualitative mask fit testing, allowing the individual to complete the entire testing process at one time.

What Do I Need to Bring with Me for a Pre-Employment Drug Screening?

All donors need to bring a photo ID and be prepared to use the restroom when arriving for their urine drug screening. Any information from your company, like a donor introduction form or passport, should also be brought to the clinic.

Who pays for the Drug Screening? Am I Required to Pay?

If a company requires a post-offer drug screening, the company will pay for the test. If the donor is voluntarily taking the drug test for their own use, GGC Employer Services expects at the time of service.

Do I Need to Bring All My Prescription Medications with Me When I Come for a Drug Test?

No. The drug test will be sent to a lab and go through a medical review process there. If needed, a Medical Review Officer (MRO) will contact the donor directly to ask about prescription medications. If the donor is also having physical examination, they will need a list of all medications for their medical history.

What Do I Bring for a Physical Abilities Test?

If you are required to take a Physical Abilities Test (PAT), it is a replication of the job for which you are being hired. You need to wear comfortable, or work appropriate clothing and sneakers or work shoes. Absolutely NO flip flops or sandals are acceptable.

Does GGC Employer Services Provide Drug Screening On-Site at My Business or Industry?

Yes. GGC Employer Services can come to your facility to conduct random drug screens or is available after hours to test for drugs and/or alcohol. Two of our Bowling Green clinics are conveniently located near two major industrial parks.

Does GGC Employer Services Accept Injured Workers from Small Employers?

Yes. GGC Employer Services serves small and large employers. We prefer to have workers comp insurance information from employers prior to treating an injured worker to expedite the treatment process. Please contact GGC Employer Services to learn more.

What Do I Bring for My Post-Offer or Annual DOT Physical?

Please bring your current DOT medical card and a list of all current medications.
